This is an example of a circular argument. So how should you treat circular arguments? It is of little use pretending they don't exist - one way to deal with them is to consciously break them up into smaller pieces so that, at least, when you report about what caused a failure, you can make the situation look clearer than it actually is. This is still difficult - you have not changed the circular form of the design process just by being selective about what you report. A better way is not to get too involved with the detailed design of failed components - you can comment, perhaps, about design features but it is often difficult to justify a categoric statement that the design of a failed component was 'wrong'.
It is perhaps the key point of effective failure investigation. Causation Causation occupies the RHS of the basic damage/causation framework shown in Fig. 2. Deciding causation is at the centre of any failure investigation and forms one of the main inputs towards the target of settlement. There is a certain thought-pattern associated with the concept of causation - to start, forget the notion that every engineering failure has, somewhere, a single, easily definable 'cause'. This is rarely true; experience shows that failures are almost always the result of a combination of conditions and 'events' - even simple failures follow this pattern.

Don't worry too much about the accuracy of this with respect to your technical role - the bigger picture is, anyway, more the domain of your clients, rather than their technical advisers.
